Once the product is created, go to Products on the left-hand panel.
Click into your Product.
Head to the Payment form tab.
Here you can either add an existing Membership Field, a Custom Form Field, or alternatively use a pre-existing form that you have already used for another product which you have previously created.
What is a Membership Field?
Membership Fields are created within the Membership section of your club control panel on the page named Fields.

How do I create a Custom Form Field?
- Go to your club control panel.
- Click Shop and payments from the left-hand panel.
- Select Products.
- Select your Product.
- Go to the Payment form tab.
- Click Add a custom form field.
- Select the Field Type.
- Enter the Name.
- Enter a Summary, this is optional.
- Select Add field.
A single line text field would be used for one word or number answers.
A multi-line text field would be used for answers that require multiple worded answers i.e. medical conditions etc.
The radio buttons option would be used for a multiple choice style answer with not too many to select from.
A drop-down field would be used for a multiple choice style answer with a greater amount of options so that it will not take up a large amount of the payments form page.
